"Share the Care" is a program established to provide wound assessment for nursing home residents. We work with the nursing homes to develop a plan of care that can be managed in the facility.
The goal is to close the wound as quickly as possible and recommend interventions to prevent further problems.
We treat the whole patient, not just the "hole" in the patient.
Treatment provided for the following wounds:
- Non-healing wounds
- Decubitus Ulcers
- Venous Ulcers
- Diabetic Feet
- Ostomy Care
- Peripheral Vascular Disease (PVD)
- Peripheral Arterial Disease (PAD)
