Ozarks Community Hospital in Gravette, AR has an immediate opening for a full-time Community Health Worker within our Swing Bed Department.

Job Summary: Under the management of the Chief Nursing Officer (CNO) and the ADON, the Community Health Worker (CHW) serves as a vital link between the hospital, patients, and community resources. The CHW assists in improving the quality of care for patients by assisting with discharge planning, managing durable medical equipment, scheduling follow-up appointments, and facilitating prescription medication pick-up.
Requirements:
- High school diploma or equivalent.
- Successful completion of certified nursing assistant program or has a significant amount of nursing assistant experience in the hospital setting, preferred.
- CHW certification, preferred.
- Valid Driver’s License required. This position may require a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment.
- Must have a valid passenger endorsement license upon hire or obtained during orientation period.
- Must hold a current Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross. Recertification required before expiration throughout employment.
